12 point offenses (resulting in 400 % surcharge)

· Manslaughter or negligent homicide
· Pre-arranged highway racing or lending car for such purpose
· Driving with blood alcohol level of .08 or more (DWI)
· Driving a commercial motor vehicle with a blood alcohol concentration of .04 or more
· Hit and Run involving bodily injury or death
· Transporting illegal intoxicating liquor for sale

10 point offenses (resulting in 300% surcharge)

· Highway racing or lending vehicle for such purpose, where the race is not prearranged
· Speeding to elude arrest (any speed)

8 point offenses (resulting in 220% surcharge)

· Driving during a period of revocation or suspension of license or registration
· Aggressive driving

4 point offenses (resulting in 90% surcharge)

· Hit and run involving property damage only
· Reckless driving
· Passing a stopped school bus
· Speeding in excess of 75 mph where the posted speed is less than 70 mph
· Speeding in excess of 80 mph where the posted speed is 70 mph or more
· Driving after consuming alcohol or drugs where the driver is less than 21 years old

2 point offenses (resulting in 45% surcharge)

· Illegal passing
· Following too closely
· Driving on wrong side of the road
· Speeing more than 10 mph over posted speed limit, where driver’s speed was in excess of 55 mph but less than 76 mph
· Speeding 10 mph or less in a posted speed zone of 55 mph or greater

1 point offenses (resulting in 25% surcharge)

· Speeding 10 mph or less where the posted speed is less than 55 mph
· Any other conviction of a moving violation
